Monterey Mushrooms MPN, #0298, was approved by the California Division of Worker’s Compensation (DWC) on March 8, 2005.  The MPN utilizes the Anthem network. Unless you pre-designate a physician or medical group, your work injuries will be treated by providers in the Monterey Mushrooms, Inc. MPN.  Those providers can be found on www.anthemwc.com.

FINDING A DOCTOR: If you have a work related injury and need to find a provider in the MPN, the following sources are available to assist you.

  • Medical Access Assistant: You may contact a medical access assistant at 1844-676-4356 Monday through Saturday from 7am to 8 pm.  The medical access assistant is available to assist you with medical access within the MPN.  The medical access assistant can assist you in finding a provider within your geographical area, assist in contacting a physician office and scheduling an appointment within the network. They can also assist with any questions or concerns.

 

  • Claims Examiner: If you need assistance with your medical care, you can contact your claims examiner at 1800-559-9891 M-F from 8 to 5pm pacific time.

 

  • Providers can be accessed via www.anthemwc.com.  Once at the main website, select California, and then you can use the tab marked provider finder to find a doctor, specialist or facility

 

  • Your local HR office can assist you in finding a provider. Regional listings of providers are linked below.

What is MPN?

A Medical Provider Network (MPN) is group of health care providers (physicians and other medical providers) used by Monterey Mushrooms, Inc. to treat workers  injured on the job. Each MPN must include a mix of doctors specializing in work-related injuries and doctors with expertise in general areas of medicine. MPNs must allow employees to have a choice of provider(s).

What if I get injured at work?

In case of an emergency, you should call 911 or go to the closest emergency room.


If you are injured at work,notify your employer as soon as possible. Your employer will provide you with a claim form.  When you notify your employer that you have had a work-related injury, your employer or insurer will make an initial appointment with a doctor in the MPN.
